Definition
Gynecologic hemorrhageuncontrolled bleeding fromvagina not associated withmenstrual period or cycle. Gynecologic hemorrhage ismedical emergency.
Causes
Trauma such as from penetrating injury or sexual assault. Ectopic pregnancy that has ruptured. Internal injuries fromblow toabdomen, particularlypregnancy. Rarely, some formsuterine cancer.
Diagnosis
Bleedingquantities greater than six ounces (halfcansoda) fromgroin that does not slow or stop. Note thatmany culturespatient will attempthide their injuries outembarrassment or fear.
First Aid
If at all possible care should be given bysame-gender individual.
Inurban setting, callhelp immediately. Apply direct pressure tooutside ofvaginal cavity. Do not attemptpackvaginal cavitycotton or any other item or object.
In wilderness first aid, immediate evacuationindicated.
Field Care (for EMTs)
Transport immediatelyhospital care. "Loadgo." Follow local protocols.
Clinical Treatment
Coagulantsmuscle agents by injection. If caused by trauma or ectopic pregnancy, immediate abdominal surgery. Supportive care including blood transfusions.
